01

The system in one sentence: entitlement, care and top-up cover

Assurance Maladie opens and manages entitlement, then reimburses care against regulated reference tariffs; it does not automatically pay the entire bill. The Carte Vitale transmits information, a declared primary doctor coordinates the care pathway and complementary insurance may cover part of the balance. Coverage, upfront payment, reimbursement basis, extra billing and the amount ultimately paid are separate questions and should be checked separately.

02

Open health rights after arrival or a change of status

France’s universal health protection, Puma, gives personal coverage to people who work in France or reside there on a stable and regular basis. The route still depends on status: employee, self-employed worker, student, pensioner, family member, returnee or person insured by another European state. An S1 form can keep the competent state responsible in certain cross-border cases. Confirm the correct scheme before filing with CPAM and retain dated copies of every document.

03

Entitlement certificate, social-security number and Carte Vitale

The attestation de droits proves coverage; the Carte Vitale helps transmit claims and reimbursements but does not create entitlement by itself. A temporary number or pending file can delay an ameli account or card without necessarily cancelling rights already recognised. Once registered, check the recorded identity, address, fund, bank details and complementary insurer, download the certificate and update the card whenever entitlement or personal circumstances change.

04

Primary doctor, specialists and finding a professional

From age 16, declaring a médecin traitant keeps a patient within the coordinated care pathway and avoids reduced reimbursement. Some consultations allow specific direct access under defined conditions, including areas of gynaecology, ophthalmology, psychiatry and neuropsychiatry; this does not always remove the need to have a doctor declared. Before booking, use the official health directory to check sector, tariff, possible Optam membership, opening hours, address and accessibility.

05

Read a reimbursement without confusing rate and bill

Under the tariffs verified by Service-Public on 7 April 2026, a €30 consultation with a contracted sector-1 general practitioner is covered at 70% of the reference price, or €21, before the €2 flat contribution is deducted: reimbursement is €19 before any complementary cover. A sector-2 doctor may charge more, but extra billing does not automatically raise the reimbursement basis. Going outside the coordinated pathway can reduce reimbursement further.

06

Complementary insurance, CSS and the meaning of 100%

A mutuelle or complementary insurer pays according to its contract: a percentage of the reference tariff, fixed annual allowance, network, waiting rule or ceiling. A “100%” label therefore does not always mean no out-of-pocket cost, particularly with extra fees, medical deductibles or services outside the reference basis. Complémentaire santé solidaire depends on resources and residence. Full ALD coverage is limited to care linked to the long-term condition and the Social Security tariff.

07

Pharmacy, hospital and emergency: use the right door

A pharmacy can advise on common situations and dispense prescriptions; the primary doctor or a local coordinated-care organisation handles non-emergency follow-up. Before planned hospital care, ask about the estimate, practitioner sector, daily hospital charge, private room and possible extra fees. For a serious medical emergency call 15 or 112; 18 covers fire and rescue, while 114 is accessible to deaf, hard-of-hearing, deafblind and aphasic users. Never delay an emergency call to check reimbursement.

08

Visitors, students and stalled applications

The European Health Insurance Card covers medically necessary care during a temporary stay under applicable rules, not planned treatment or every expense. Visitors outside European coordination should check travel cover, exclusions and upfront-payment requirements. International students must follow the official route matching nationality and status. If a file stalls, record dates, acknowledgements, invoices and paper claims; request a written response from the fund before using a formal complaint or mediation route.

FRA9 · FAQ

Frequently asked questions

Is the Carte Vitale health insurance?

No. It transmits insured-person information and facilitates reimbursement; the entitlement certificate proves coverage.

Must a primary doctor be declared?

From age 16, declaring one keeps care coordinated and avoids reduced reimbursement, subject to defined exceptions.

Is complementary health insurance mandatory?

It is not universally mandatory for individuals, but private employers normally offer collective cover, with permitted exemptions.

What does 100% reimbursement mean?

It usually means 100% of the Social Security reference basis, not necessarily the full bill or extra fees.

Which number should be called for a serious medical emergency?

Call 15 or 112; 114 is the accessible emergency service. Do not delay the call to investigate cost.
